Ceramides — it's the ratio, not the amount

More ceramide isn't automatically better. In aged skin, a cholesterol-dominant lipid mix sped barrier recovery — and which lipid should lead depends on what that skin is short of.

A higher ceramide percentage doesn't automatically repair the barrier faster — what matters is how the three barrier lipids sit in proportion to each other, which no single percentage tells you. The barrier is built from ceramides, cholesterol, and fatty acids arranged in layers. In chronologically aged skin, a mixture with cholesterol as the dominant lipid accelerated barrier recovery, and which lipid should lead depends on what that skin is short of (Zettersten et al., 1997). So "how much ceramide" is the wrong question to lead with.

Ingredient lists invite the amount question, because a percentage is easy to print and easy to compare. Structure is harder to advertise and matters more.

What is the barrier actually made of?

Three lipid families, stacked. Ceramides, cholesterol, and free fatty acids sit between the cells of the outer layer in ordered sheets, and that arrangement is what holds water in and keeps irritants out. The lipids only work as a barrier when they're organized, not merely present. Does more ceramide mean faster repair?

Not on its own. When the lipids were applied in different proportions, a 3:1:1:1 mixture with cholesterol dominant sped recovery in aged skin; in the animal arm of the same study, making fatty acids dominant slowed it instead (Zettersten et al., 1997). There are four numbers because the free fatty acids are counted as two — essential and non-essential — and the leading "3" is simply whichever lipid dominates, not always ceramide as the market's "3:1:1" implies. The same ingredients, reordered, produced opposite results — which is why a ceramide percentage alone tells you very little.

Does this only apply to older skin?

No — the ratio matters at any age. In young skin an equimolar mixture allowed normal recovery and any 3:1:1:1 ratio sped it up (Zettersten et al., 1997). What changes with age is which lipid belongs in the "3", because what's short changes. Aged skin is relatively low in cholesterol, so a mixture leaning that way replaces what's actually missing rather than adding more of what's already there. That's the practical reading of the finding: match the shortage, don't maximize a single number. A formula is worth judging by whether it supplies all three lipids in a sensible proportion, not by one headline percentage.

How do you read a product label, then?

Look for all three families present rather than a single starred ingredient, and treat a large ceramide number with the same skepticism as any other headline claim. Then judge by how skin behaves over a few weeks — less tightness, fewer stinging days. A barrier product is doing its job when skin stops needing rescuing, not when a number on the box is impressive. FAQ

Is a high ceramide percentage a bad sign?
Not bad, just incomplete. What drove recovery speed was the proportion between the three lipids, not the amount of any one (Zettersten et al., 1997).
Which lipid should dominate?
In chronologically aged skin, a cholesterol-dominant mixture sped recovery (Zettersten et al., 1997). The human part of that study was small — six people — so it isn't a universal rule for every product.
Do I need a ceramide product at all?
Not necessarily. If skin isn't tight or stinging, a simple moisturizer is enough. Barrier lipids matter most while skin is recovering.
Can I layer several barrier creams?
Layering more products adds more chances to irritate. One well-composed moisturizer used consistently does more than three used occasionally.
